Who is a Good Candidate for a BBL Combined with a Tummy Tuck?

'Hey, what's up guys? It’s Dr. McDaniel. So, we've gotten a couple of questions asking: 'Who's a good candidate for a BBL combined with a Tummy Tuck?' How do you know if that's something you would benefit from, and what can you expect when combining these surgeries?
- The person who benefits the most from this is someone who, truthfully, has pretty good shape to begin with, but they have a little bit of loose skin. And oftentimes, doing liposuction with loose skin turns out actually really poorly because the skin oftentimes won't contract enough, and you end up having more flaccid skin.
- And then the tummy tuck becomes harder because you can easily develop fibrosis underneath it. So later, if you come back to do it, it can be more challenging and have irregular contour. So, the person who again benefits from it is one who hasn't had a lot of liposuction on the stomach, hasn't had a—or fibrosis that’s developed from previous lipo—and it’s someone who has a little bit of loose skin.
- So, this patient (case 1)— you can see the irregular contour of the skin here, and that is primarily from having children, having various weight changes, gaining weight, losing weight. And now if you lipo this, this is going to be very flaccid. It’s not going to look good. She’ll be thin in clothing, but outside of clothing, I don't think that she'll be happy.
- So, she’s a great candidate to combine this because not only can I tighten the skin, give what looks to be Ab etching or some type of high-definition liposuction—but that's actually just the suturing; there’s no liposuction here. It's actually very unsafe to do tummy tucks —full tummy tucks— with high-definition liposuction.
- The reason it's good to combine hers is because now you can see this slight hip dip here, as well as the fullness in her waist. Now we’ve brought her waist in, but I use the analogy of being able—when I’m combining the surgery—using an eraser and a pencil. So now with liposuction, I can 'erase' some of the volume, and with tummy tuck, remove some of that skin to 'erase' it, but without having that 'pencil' to draw in and fill in that deficiency in the hips—which is the BBL—she’s not going to have as good of a shape.
So now I can have two points of control: one, removing tissue, skin, and volume and fat in the sides; and then adding fat here in the lateral aspect of the hips to really give her that silhouette shape. So, she’s a perfect candidate for someone who should be combining this surgery. And you can see a phenomenal change. This is day one after surgery, so this is going to look better with time.
Another good example (case 2): this person is a little bit further out from surgery, I want to say she’s a couple of weeks here. So you can really see the change in the silhouette and the smoothness of that. Now, she had pretty good shape to begin with, but if I had just liposuctioned this area, number one, it would be flaccid, and the height of her hips is still pretty low. Here we’ve brought it up some because I've added the volume.
- And then this last example (case 3) is another example of someone who really benefited. Similar to the first case I showed, you can see how it has some concavity here, or flatness at minimum, and then she has a little bit of loose skin. If I zoom in on the skin, you can actually see this striae or stretch marks; that's a tell-tale sign of someone who’s had damage and that thinning of the skin. It’s just not going to contract as compared to someone who doesn't have that damage to the skin. So the best way to remove that in most patients is to actually do a tummy tuck, get rid of the skin, and then add the volume laterally to the hips.
